Surgery clinics

We offer a range of clinics and services at the surgery. There are some tasks you can do, such as health reviews, using our online forms or the Accurx service.

Asthma/COPD management

If you have been advised by the surgery to submit an annual review of your asthma/COPD symptoms, please complete our asthma review (adults) form or our COPD assessment form.

Diabetes management

If you have been advised by the practice to complete a diabetes review, please complete our diabetes review form.

Smoking cessation

If you have been advised by the surgery to submit smoking reviews on a regular basis, please complete our smoking review form.

Cervical smears

The practice offers a cervical smear screening service, and all women between the ages of 25 and 60 will automatically be invited every three to five years to attend.

The test results are usually available after about 2 weeks.

We therefore urge all women between 25 and 65 who have not had a smear test in the last five years to ensure that they book one as soon as possible with the practice nurse.

Family planning advice

Long term conditions

We are improving the way we work with patients with long-term conditions

Patients living with certain long-term conditions will be encouraged to attend a yearly health check. Long-term conditions are those that impact over a long period of time, such as diabetes and heart diseases.

Patients will receive personalised care and support from healthcare professionals from their general practice. This will cover things that patients say matter most to their health and wellbeing, from the best treatment for their condition, to wider things like employment, housing, and mental health.

Interpreting services

If you think you may need help with translation, you are very welcome to bring a friend or relative with you (18 and over only please!). Alternatively, let our reception staff know when you book and ask for a double appointment. All our doctors and nurses have access to the “language line” telephone translation service to help you.

Several staff speak a second language and may also be able to help with translation, but this will depend on their availability when you come to the surgery. Languages spoken include Bengali, Gujerati, Punjabi, Turkish and Urdu.

If you would like a British Sign Language interpreter, we are very happy to book one for you. Please give us as much notice as your problem allows, as sign language interpreters are often very busy.
